Our aim is to help make life easier for you. We will assist you, however we can ensure you, that we have up to date records. This is to ensure compliance with law and protection for you.
To assist you with obtaining adequate medical records we have created this section.
Please ensure that you attach clearly mark your membership number on any faxed emailed or posted communication
To use our service you must have

Had a physical examination in the last 18 months |

In order for us to maintain legal guidelines, you must fax evidence of your existing medical condition, prior to your 1st Refill. The doctors require documents that substantiate the condition(s) for which you are consulting and must not be more that 1 year old. We do not need your entire medical history. |

Print out the Physical exam form and take it to a Doctor and ask him to complete and sign
